Bayview Hunters Point Foundation provides Enhanced Care Management (ECM) services to support individuals with complex medical, behavioral health, housing, and social service needs. Through a person-centered, trauma-informed approach, our care teams work closely with clients to coordinate healthcare, behavioral health treatment, housing resources, benefits enrollment, and other community-based services.

Our ECM program focuses on improving health outcomes, increasing access to care, reducing barriers to stability, and helping individuals navigate complex systems. By building strong relationships with clients and community partners, we help individuals achieve greater self-sufficiency, improve their overall well-being, and maintain long-term stability within their communities.
